About this form
This form is intended to collect information pertinent to you Will appointment. Please leave us your identification and contact details in the form below and one of our Customer Service Officers will contact you to arrange an appointment. Executors are the persons charged with carrying out the instructions set out in your Will. The Public Trustee and Guardian is only permitted to prepare your Will if you appoint the Public Trustee and Guardian as your executor or as an alternate executor. In addition to drafting your Will, we recommend that you also have an up to date Enduring Power of Attorney.
At your first interview you will be required to identify yourself and you may also be asked questions relating to your capacity to make a Will.
