Medicines, Poisons and Therapeutic Goods New Licence Application
About this form
This form is to be used to apply for a licence under the Medicines, Poisons and Therapeutic Goods Act 2008 (the Act).
A medicines and poisons licence is required by individuals or corporations to authorise dealings with a regulated substance when they are not otherwise authorised under legislation to deal with the regulated substance.
The applicant should be familiar with the Act and the Medicines, Poisons and Therapeutic Goods Regulation 2008.
Opioid dependency treatment licence (pharmacist): The applicant should also be familiar with the National Guidelines for Medication-Assisted Treatment of Opioid Dependence (2014) and the training requirements that are outlined in the Medicines, Poisons and Therapeutic Goods (Guidelines for treatment of opioid dependency) Approval (No X).
Failure to comply with ACT legislation renders a person liable to prosecution.
