Request for Transcription of Immunisation Records

About this form

This form is used to request that ACT Health transcribe vaccines your child has received while overseas.

If your child has received vaccines overseas, these will need to be recorded on the Australian Immunisation Register. Your child may require additional vaccines to meet the Australian National Immunisation Program schedule.

To use this service, your child must be under 10 years of age and your child's immunisation history must be in English, or have an English translation. ACT Health is unable to translate immunisation records. If you need your documents translated, please contact a translation centre.

Your general practitioner may be able to transcribe your child's overseas immunisation history and provide a catch up schedule if required. Your general practitioner can also advise you on vaccinations required for your child to meet the Australian National Immunisation Program schedule.

There is no fee for this service.
